Folate and Folic Acid Dosage: How Much Per Day?

Folate is the nutrient where the units cause more confusion than the biology. Dietary folate equivalents, micrograms of folic acid and milligrams of methylfolate all appear on labels, and they are not the same measurement.

DFE, folic acid and methylfolate are three different units

Folate is the general term for the B9 family. Folic acid is the stable synthetic form used in supplements and food fortification. 5-methyltetrahydrofolate — sold as methylfolate, L-5-MTHF or L-methylfolate — is the circulating form the body uses.

Because folic acid is absorbed more efficiently than food folate, requirements are expressed in dietary folate equivalents. One microgram of DFE equals one microgram of food folate, but only about 0.6 micrograms of folic acid taken with food. So a 400 microgram folic acid tablet supplies roughly 667 micrograms DFE, which is why the requirement figure and the label figure never match neatly.

The upper limit, in contrast, is expressed in plain micrograms of folic acid, not DFE. A 1,000 microgram folic acid tablet sits exactly at the adult ceiling before you count fortified bread, cereal and pasta.

The pregnancy evidence is the strongest in the whole supplement field

Randomised trial evidence that periconceptional folic acid reduces neural tube defects is unusually strong, and it drove mandatory fortification of grain products in the United States from 1998. The CDC advises 400 micrograms of folic acid a day for anyone who could become pregnant, starting at least a month before conception, because the neural tube closes in the first weeks — often before a pregnancy is known.

That timing detail is the reason the advice is framed around anyone who could become pregnant rather than anyone who is pregnant. Starting at the positive test is already late for the specific outcome the evidence supports.

Higher amounts are advised in specific circumstances — a previous pregnancy affected by a neural tube defect, certain anticonvulsants, some malabsorptive conditions — but those figures come from a clinician who knows the history, not from a shelf.

Masking B12 deficiency: the reason for the ceiling

High folic acid intake can correct the anaemia of vitamin B12 deficiency while the neurological damage continues unchecked. The NIH ODS is explicit about this: the 1,000 microgram upper limit exists largely to reduce the risk of masking a B12 deficiency that would otherwise be caught by a blood count.

This matters most in older adults, in people on metformin or long-term proton pump inhibitors, and in vegans — the same groups with elevated B12 deficiency risk. Taking a high-dose folate supplement without knowing your B12 status is the specific pattern to avoid.

If you supplement folate at high amounts long term, having B12 checked is a reasonable precaution rather than an anxious one.

MTHFR variants and the methylfolate question

Common variants in the MTHFR gene, particularly C677T, reduce the activity of the enzyme that converts folic acid to the active methylfolate form. This is the basis for marketing methylfolate as superior for people carrying those variants.

What is established is that the variant affects enzyme activity and homocysteine levels. What is not established is that people with the variant need a different supplement to achieve adequate folate status — most reach normal levels on ordinary folic acid, and public health advice for pregnancy still specifies folic acid because that is the form the trial evidence used.

Methylfolate is a legitimate product and generally well tolerated. Treat the claim that it is necessary because of a genetic test result as commercial rather than settled science, and be aware that direct-to-consumer MTHFR testing is not recommended by major genetics bodies for this purpose.

Food sources, medicines and who runs low

Dark leafy greens, legumes, asparagus, brussels sprouts, avocado, liver and fortified grain products are the main sources. In countries with mandatory fortification, background intake from bread and cereal is substantial and should be counted before adding a supplement.

Low folate status concentrates in people with coeliac disease or other malabsorption, people with heavy alcohol use, people on methotrexate or certain anticonvulsants, and pregnancy where requirements rise.

Methotrexate deserves particular attention. It works partly by blocking folate metabolism, and folic acid is often prescribed alongside it in rheumatology to reduce side effects — but in oncology the interaction can matter differently. Never adjust folate around methotrexate without the prescribing team's instruction.
