Postmenopausal HSDD — best-quality women's RCT

Vale et al. (J Sex Med 2018) randomised 60 postmenopausal women with HSDD to Tribulus terrestris 750 mg/day (standardized) vs placebo for 120 days. The Tribulus arm showed statistically significant improvements in Female Sexual Function Index (FSFI) desire, arousal, lubrication, orgasm, and satisfaction domains, and reductions in Female Sexual Distress Scale scores. Total and free testosterone did not rise, consistent with a non-androgenic mechanism.

Premenopausal HSDD

Akhtari et al. (Daru 2014) tested Tribulus 7.5 mg/kg/day (roughly 500 mg for a 65-kg woman) vs placebo in premenopausal women with HSDD for 4 weeks.
