What the research shows
Postmenopausal insulin sensitivity (Yoshino 2021)
A 12-week randomized, placebo-controlled trial in 25 postmenopausal women with prediabetes (Yoshino et al., Science 2021) reported a ~25% relative increase in muscle insulin sensitivity (measured by hyperinsulinemic-euglycemic clamp) at 250 mg NMN/day. However, downstream markers — HbA1c, fasting glucose, body composition — did not shift meaningfully. The trial is often cited as a positive result, but the absolute effect sizes are modest and the sample is small.
Dose-ranging safety and NAD+ pharmacokinetics (Yi 2023)
A 60-day parallel-group study in 80 middle-aged adults (Yi et al., GeroScience 2023) compared 300, 600, and 900 mg NMN daily. Whole-blood NAD+ rose dose-dependently (roughly +40% at 300 mg, +80% at 900 mg vs baseline). Six-minute walk distance improved modestly at 600 and 900 mg. No serious adverse events; a small number of participants reported mild GI symptoms at the highest dose.
Physical performance in older adults (Igarashi 2022)
A 12-week Japanese RCT (Igarashi et al., NPJ Aging 2022) of 250 mg NMN in adults aged ≥65 found improvements in gait speed and grip strength when NMN was taken in the afternoon rather than morning — an unexplained timing finding that has not been replicated. It highlights how thin and variable the current NMN human trial base is.
Cardiometabolic markers (Katayoshi 2023)
A 12-week trial in Japanese adults with elevated LDL (Katayoshi et al., Scientific Reports 2023) tested 250 mg NMN daily. LDL and diastolic BP fell modestly vs placebo; the effect size was small (a few mg/dL and ~2 mmHg). Whether this translates to fewer cardiovascular events over years is unknown — no NMN trial has been long enough or large enough to answer that.
NAD+ vs downstream outcome disconnect
Every NMN trial that measures blood NAD+ shows the biomarker rises. Far fewer trials show a matching downstream benefit at the clinical level.
